In gaming, False PHD is a passive item added in the Repentance DLC of the roguelike game The Binding of Isaac: Rebirth.
Unlike the standard PHD item which guarantees beneficial pill outcomes, False PHD reverses the strategy by leaning entirely into negative pills while rewarding you with significant stat and health bonuses. Core Effects
Pill Identification: Identifies the effect of all pills upon pickup, removing the risk of blindly swallowing unknown pills.
Immediate Health: Grants one Black Heart immediately when you pick it up.
Negative Conversion: Converts all “good” pills into their “bad” counterparts within your pill rotation. Secondary Bonuses (The Twist)
While it forces you to take bad pills, it adds highly beneficial secondary mechanics whenever you consume them:
Stat Down Pills: Every time you swallow a pill that decreases a stat (e.g., Tears Down, Range Down, Luck Down), it simultaneously grants you a permanent Damage Up (+0.6 DMG).
Non-Stat Negative Pills: Swallowing non-stat negative pills (such as Amnesia, Addicted!, or Paralysis) rewards you with a Black Heart.
Neutral Pills: Purely flavor or spatial neutral pills (like X-Lax or I Found Pills) have no secondary effect and grant no bonuses. Notable Item Synergies
